The role of the Community Health Trust

Shropshire Community Health Trust serves patients across Shropshire, Telford & Wrekin. The services provided range from district nursing and health visiting, to physiotherapy and specialist community clinics.

The Community Health Trust forms a key part of the NHS Future Fit programme and their services will form a significant part of the clinical model that NHS Future Fit is proposing. The home visiting services will support the clinical model of people receiving more of their care closer to home. This may be following an operation or procedure in hospital, or to help support people to remain independent if they have long term illnesses or conditions such as diabetes, dementia or cancer.  Our services will also play an important role in keeping patients well and being able to respond rapidly to unexpected changes in a patient’s health. There is recognition that we need to join up health and social care services across Shropshire, Telford & Wrekin and we know that this will work best if doctors, nurses and other health and care professionals working in the community can communicate easily with specialists who are based in the hospitals. By working more closely, in joined up teams, they will be better able to keep patients at home and help them to manage their health safely.
